Q:What information should I bring when I come to Jamaica Hospital?
A:Each patient should bring the items below when he or she registers:
Driver’s license
Insurance card
Referral form from your doctor (if your insurance company requires this)
Advance directives
A list of medications that you are currently taking

Q:What are the visiting hours/ restrictions for patients?
A:The visiting hours vary depending on the units in which patients are staying. In addition, restrictions may range from safety concerns to a court order restricting contact. Please see the visiting information for more details.

Q:Where Can I Park?
A:We have a safe and secure 10 level parking garage for your comfort and convenience. To reach the parking lot, turn right on 89th Avenue. This reasonably priced facility is monitored by camera and security patrols and is a good alternative to parking on the local street. Entrance to the parking garage is halfway down the block on the right.

Q:How Do I Locate A Patient In the Hospital?
A:Please contact Jamaica Hospital’s Admitting Department at 718-206-6075, and provide the correct and full name of the patient.

Q:How do I obtain my medical records?
A:You can obtain full instructions from this website. From the home page, please click on “FOR OUR PATIENTS” on the top menu bar, then the “MEDICAL RECORDS” tab to learn how to request your medical records online or in person.

Q:How do I obtain the birth certificate and Social Security card for my child?
A:Jamaica Hospital doesn’t issue any birth certificates. Instead, the New York State Department of Health sends out birth certificates. If you would like your child’s birth certificate, call the health department at (212) 788-4520. You can also request a replacement for a lost certificate at https://www.health.ny.gov/vital_records/birth.htm.

Your child’s Social Security card should arrive in the mail within 3-4 weeks after your baby’s birth. We initiate this process at the hospital, and you don’t need to complete and submit any other documents outside of our hospital as part of this process.
